Mobile clinic for Thai passports

PHUKET CITY: The Consular Affairs department of the Thai Ministry of Foreign Affairs will hold a mobile service center for Thai nationals to apply for “e-passports” in Phuket City from May 12 to 23.

The service center, located in the conference room of the Phuket Provincial Administration Organization (OrBorJor) offices on Narisorn Rd, will be open from 8 am to 4:30 pm daily, closed only on Sunday, May 18.

An e-passport, which is valid for five years, costs 1,000 baht, plus 35 baht to cover postage. Passports will be delivered about one week after application.

Adult applicants must present their identification card, government officer card or state census card.

Applicants under 20 years old must be accompanied by at least one parent. If both parents are present, they both must present their original ID cards.

In case one parent cannot be present, the parent applying for the minor’s passport must present the original ID card of the absent parent and an agreement signed by the absent parent to let the child out of the country. The agreement must be ratified by a District Office.

If the minor’s parents have divorced, the parent applying with the minor must provide documents proving the parent’s right as guardian of the minor. The guardian must also present his or her original ID card.

Applicants will also be required to provide documentation of any changes of name.
